INTRODUCTION

As an advanced practice nurse, it is important to diagnose, treat, and evaluate patients who have chronic disease. A nurse must understand how pathology, treatment, regimens, and psycho-social issues affect patients and the care they receive. Disease management is more than just monitoring a medication or treatment; it is evaluating the disease process from the micro to macro level. As a nurse with an advanced degree, you will be expected to assess patients and individual and population responses to chronic illness.

For this assessment, you will investigate pathopharmacological issues related to a specific disease process. You may choose to investigate hypertension. As part of this assessment, you will analyze the various impacts the disease process has on the patients, their families, and populations at a local, national, and international level.

REQUIREMENTS

Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. An originality report is provided when you submit your task that can be used as a guide.

Professional Communications is a required aspect to pass this task. Completion of a spell check and grammar check prior to submitting your final work is strongly recommended.

You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.

  1.  Investigate the following disease processe: hypertension
  2.  Analyze the pathophysiology of the disease process you selected in part A.
  3.  Discuss the standard of practice for the selected disease process.
  4.  Discuss the evidence-based pharmacological treatments in your state and how they affect management of the selected disease in your community.
  5.  Discuss clinical guidelines for assessment, diagnosis, and patient education for the selected disease process.
  6.  Compare the standard practice for managing the disease within your community with state or national practices.
  7.  Discuss characteristics of and resources for a patient who manages the selected disease well, including access to care, treatment options, life expectancy, and outcomes.
  8.  Analyze disparities between management of the selected disease on a national and international level.
  9.  Discuss three or four factors (e.g., financial resources, access to care, insured/uninsured, Medicare/Medicaid) that contribute to a patient being able to manage the selected disease.
  10.  Explain how a lack of the factors discussed in part A4 leads to an unmanaged disease process.
  11.  Describe characteristics of a patient with the selected disease that is unmanaged.
  12.  Analyze how the selected disease process affects patients, families, and populations in your community.
  13.  Discuss the financial costs associated with the selected disease process for patients, families, and populations from diagnosis to treatment.
  14.  Discuss how you will promote best practices for managing the selected disease in your current healthcare organization.
  15.  Discussthree strategies you could use to implement best practices for managing the selected disease in your current healthcare organization.
  16.  Discuss an appropriate method to evaluate the implementation of each of the strategies from part C1.
  17.  When you use sources, include all in-text citations and references in APA format.

Note: When using sources to support ideas and elements in an assessment, the submission MUST include APA formatted in-text citations with a corresponding reference list for any direct quotes or paraphrasing. Leadership Styles and Nursing

Leadership Styles and Nursing

This is a Collaborative Learning Community (CLC) assignment.
The purpose of this assignment is to assess leadership styles, traits, and practices as a nursing professional, establish the importance of effective interprofessional communication as a leader in nursing, and to explore the role of servant leadership in nursing practice.
Read the study materials on leadership and complete the topic quiz activities to better understand your leadership qualities.
Upon completion, summarize and share with your group what you learned about your specific leadership qualities, so you can become familiar with how you are similar and different from your peers when it comes to being a leader.
As a group, review the study materials related to servant leadership. Using what you have learned about the tenets of servant leadership and traits and practices of successful leaders, create a 10-12 slide PowerPoint presentation with speaker notes. Add an additional slide for
references at the end of your presentation. Include the following in your presentation:

4 Discuss what leadership traits and styles are necessary to be an effective communicator.
Explain the importance of leaders adapting communication approaches when working interprofessionally (across ancillary departments, vendors, community members).
5 Discuss how nursing professionals can benefit from integrating the tenets of servant leadership to empower and influence others as they lead.

6 Discuss how leaders who practice servant leadership and have a strong understanding of their personal leadership traits can successfully lead others and navigate the unique challenges that are part of nursing and health care. Provide two examples that illustrate your main ideas. You are required to cite to a minimum of three sources to complete this assignment. Sources
must be published within the last 5 years and appropriate for the assignment criteria and relevant to nursing practice.

Module #7: Final Paper Assignment

INSTRUCTIONS

Write your paper in a Word document using APA style. Consult the exemplar papers included in the Module 7 readings and the examples at the end of each chapter of the textbook. It is highly recommended that you use the headings and subheadings suggested below. You may add additional headings or subheadings if needed. Refrain from the overuse of quoted material; instead paraphrase the content into your own words and properly cite. Save direct quotes for powerful passages. Consult the grading rubric to be sure you address the key topics. As always, include citations to support your work. It is expected that you will have at minimum of 10 citations – you may have many more. There is no absolute page requirement; your goal should be to write concisely and clearly.  All papers will be submitted via the Turnitin program. The final paper is worth 35% of your grade – your grade on this assignment may impact your final grade positively or negatively by a full letter grade.
