Differential Diagnosis. Case Study Vignette

Case Study

A 78-year-old female comes to your office escorted by a neighbor who is a patient of yours. The
neighbor, who has lived next door to the older woman for years, relates that a week ago the
elderly woman’s sister died and that she had been her caregiver for many years. The neighbor relates that although she would occasionally see the older woman, she did not visit the home. At the funeral last week, she noticed that the woman appeared fatigued, confused, sad, and gaunt in appearance. Later the neighbor approached the woman, inquired about her health, and
determined that the woman had a very difficult time the past couple of months, caring alone for
her sister until the end when hospice care was initiated. The neighbor convinced the woman to
seek medical care and today is the first appointment with a provider that this 78-year-old female
has had in 3 years. The older woman states that she is very fatigued and sad over the loss of her
sister. Neither her sister nor the patient has been married. A distant niece came to the funeral but
lives about 30 miles away. The woman states that she is not taking any prescription medication
and relates no medical problems that she is aware of being diagnosed.
Vital signs: T 97.6°F, HR 98, RR 22, BP 95/60, BMI 21
Chief Complaint: Fatigue and sadness over the death of her older sister.
Discuss the following:
1) What additional subjective information will you be asking the patient?
2) What additional objective findings would you be examining the patient for?
3) What are the differential diagnoses that you are considering?
4) What laboratory tests will help you rule out some of the differential diagnoses?
5) What screening tools will you select to use on this patient?
6) What is your plan of care?
7) What additional patient teaching may be needed?
8) Will you be looking for a consult?

Nurses Serving on Boards

boards nurses are serving on across states and the nation. Also identify two boards in your local
community that you might consider serving on in the future – and the leadership competencies
required for those specific boards which you might need to develop or enhance.”-Professor

Physical Assessment: Child vs Adult

Instructions

Compare the physical assessment of a child to that of an adult.

In addition to describing the similar/different aspects of the physical assessment, explain how the nurse would offer
instruction during the assessment, how communication would be adapted to offer explanations,
and what strategies the nurse would use to encourage engagement.

Homeostasis

This is a short essay answering the following two questions.

Instructions

Compare and contrast two leadership styles or theories

Mini-Paper: Compare and Contrast two leadership styles or theories

This Assignment fulfills the following course objective:

  1. Integrate nursing leadership concepts and theories into the various roles of the professional registered nurse to promote safe and high quality nursing care.

In this assignment you will select two different leadership styles or theories, and write a 2-3 page paper comparing/contrasting the two. Focus on how the two are similar to each other (comparison), and then describe how they differ (contrast). When you are writing this paper, reflect on analyzing the differences, how and under what conditions would they be utilized, and what is important about them. State how these would or would not be significant to the nursing profession.

Nursing Theories of Caring. Watson’s Theory of Caring

Jean Watson’s Theory of Human Caring

Summary: A theory can be used to guide practice.

This assignment is an exercise in supporting a clinical practice with theory and evidence.

Directions: Identify an outcome of nursing practice in your area of practice that can be improved.

For example, if you work in home health, you may identify that throw rug use by fall risk patients is too prevalent.

You may be able to use the problem that inspired the theory concepts that you developed in week two.

Identify the concept in Watson’s Theory of Caring that could represent or include the outcome.

In our example, the outcome would be the changes in self through the change in the patient’s floor covering practice.

Identify a practice that can be changed or implemented that may influence the outcome.

Identify the concept in Watson’s Theory of Caring that includes the practice.

In our example, the practice could be to improve the understanding of fall hazards through the engagement in a teaching-learning experience, one of Watson’s Caritas Processes.

Identify exactly how these two concepts will be measured with their operational definitions.

Develop a proposition between the two. P

resent your outcome in an APA formatted paper meeting the University’s standards for a written assignment. Length: 5 to 7 pages including title and reference pages References: 3 to 10.

There should be enough to support the links between the concepts of the problem and the concepts of Watson’s Theory of Caring.
