Secondly,  Identify a minimum of three strategies your intended target audience can use to achieve desired results on your selected topic. Use an appropriate heading and graphics to outline the steps for your intended target audience for them to achieve desired results (related to selected topic).

Thirdly,  Utilize the foundations of the behavioral theory/model in the design and content of your health education brochure. Be sure to provide a reference for this on your reference page.

Further, Create a title page for your brochure. Include your name, the target audience, and feedback from a member of your target audience.

Please note you will need to share your brochure with a member of your target audience and document their feedback on the title page.  For example, John Doe reviewed the brochure and indicated he had difficulty understanding some of the terms. The terms were modified.

You are working at a family medicine clinic with Dr. Hill. She tells you, “The next patient, Mr. Smith, is a 53-year-old male with a chief concern of swelling and pain in his left lower extremity.”

Before you go to see Mr. Smith, a quick review of the chart reveals that he has type 2 diabetes, hypertension, and hyperlipidemia. You note that he has not been to the office in the past six months, and it appears that he should be out of all of his medications.

When you enter the examination room, Mr. Smith, an obese, middle-aged male, greets you from where he is sitting. You introduce yourself and ask him what brings him to the office today.

He replies, “It’s my left leg. The past four days it has been red, swollen, and painful—and it seems to be getting worse.”
You ask him to tell you more about this problem.

He says, “It began several days ago, and the swelling seems to be getting worse. It hurts all the time; it doesn’t even get better when I rest it. Additionally, it seems to get a little worse when I move around. It hurts to walk as soon as I try to stand on it.”

REASON FOR VISIT HISTORY

You meet Mr. Smith and interview him about his leg problem.

In 2001, 18-month-old Josie King died of dehydration and a wrongly administered narcotic while an inpatient at Johns Hopkins Hospital.  Her mother, Sorrel King, tells the story of how this happened and explains her role as a consumer (parent) in improving patient safety.
