Risk Evaluation and Mitigation Strategy in Medical Setting

Risk Evaluation and Mitigation Strategy

  • Pick a pharmaceutical or biologic that is approved in the EU or US that does not presently have a REMS Risk Evaluation and Mitigation Strategy.
  • Try to choose one that has had previous safety issues as evidenced by either communications from Regulatory Authorities or SAE’s (Serious Adverse Event) reported by healthcare facilities or patients.
  • Prepare a REMS for the product that you choose following the FDA guidance document, “Format and Content of a REMS Document, dated October 2017. https://www.fda.gov/media/77846/download
  • Each REMS should contain a either a mocked-up patient enrollment form or a prescriber enrollment form.
  • Make sure that you include at least one REMS element (i.e., Medication Guide, patient package inserts or communication plan).
  • The REMS element will be separate from the REMS. Either do it as a separate document that is appended to the REMS or start it separated by a page or section break of your REMS

Note: if the drug or biologic you choose has a medication guide then you cannot provide the medication guide as your one REMS element.

You will have to choose another REMS element to provide. The REMS elements should be created by you.

Format:

Times New Roman, 12 font

Cite your references.

Due:

REMS document that includes:

  1. Include either a mocked up patient or prescriber enrollment form
  2. Include one REMS element (i.e., Medication Guide, patient package inset or communication plan)

Quality Improvement Initiatives

Prepare an 8 page data analysis and quality improvement initiative proposal based on a health issue of professional interest to you. The audience for your analysis and proposal is the nursing staff and the interprofessional team who will implement the initiative.

“A basic principle of quality measurement is: If you can’t measure it, you can’t improve it” (Agency for Healthcare Research and Quality, 2013).

Health care providers are on an endless quest to improve both care quality and patient safety. This unwavering commitment requires hospitals and care givers to increase their attention and adherence to treatment protocols to improve patient outcomes. Health informatics, along with new and improved technologies and procedures, are at the core of virtually all quality improvement initiatives. The data gathered by providers, along with process improvement models and recognized quality benchmarks, are all part of a collaborative, continuing effort. As such, it is essential that professional nurses are able to correctly interpret, and effectively communicate information revealed on dashboards that display critical care metrics.

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:

  • Competency 2: Plan quality improvement initiatives in response to routine data surveillance.
    • Outline a QI initiative proposal based on a selected health issue and supporting data analysis.
  • Competency 3: Evaluate quality improvement initiatives using sensitive and sound outcome measures.
    • Analyze data to identify a health care issue or area of concern.
  • Competency 4: Integrate interprofessional perspectives to lead quality improvements in patient safety, cost effectiveness, and work-life quality.
    • Integrate interprofessional perspectives to lead quality improvements in patient safety, cost effectiveness, and work-life quality.
  • Competency 5: Apply effective communication strategies to promote quality improvement of interprofessional care.
    • Apply effective communication strategies to promote quality improvement of interprofessional care.
    • Communicate evaluation and analysis in a professional and effective manner, writing content clearly and logically with correct use of grammar, punctuation, and spelling.

Benchmarks for Quality Indicators

These databases provide recognized benchmarks for quality indicators.

In this assessment, you will propose a quality improvement (QI) initiative proposal based on a health issue of professional interest to you. The QI initiative proposal will be based on an analysis of dashboard metrics from a health care facility.

  • Analyze data from the health care facility to identify a health care issue or area of concern. You will need access to reports and data related to care quality and patient safety. If you work in hospital setting, contact the quality management department to obtain the data you need.
  • You will need to identify basic information about the health care setting, size, and specific type of care delivery related to the topic that you identify. You are expected to abide by HIPAA compliance standards.
Option 2

If you do not have access to a dashboard or metrics related to a QI initiative proposal:

  • You may use the hospital data set provided in the media piece titled Vila Health: Data Analysis. You will analyze the data to identify a health care issue or area of concern.
  • You will follow the same instructions and provide the same deliverables as your peers who select Option 1.

Instructions

Analyze dashboard metrics related to the selected issue.

  • Provide the selected data set in the proposal.
    • Assess the stability of processes or outcomes.
    • Delineate any problematic variations or performance failures.
  • Evaluate QI initiatives on the selected health issue with existing quality indicators from other facilities, government agencies, and non-governmental bodies on quality improvement.
    • Analyze challenges that meeting prescribed benchmarks can pose for a heath care organization and the interprofessional team.
  • Outline a QI initiative proposal based on the selected health issue and data analysis.
    • Identify target areas for improvement.
    • Define what processes can be modified to improve outcomes.
    • Propose strategies to improve quality.
    • Define interprofessional roles and responsibilities as they relate to the QI initiative.
    • Provide recommendations for effective communication strategies for the interprofessional team to ensure the success of the QI initiative. Briefly reflect on the impact of the proposed initiative on work-life quality of the nursing staff and interprofessional team.
  • Integrate relevant sources to support arguments, correctly formatting citations and references using current APA style.

The numbered points below correspond to grading criteria in the scoring guide. The bullets below each grading criterion further delineate tasks to fulfill the assessment requirements. QI data

Hi! I heard through word of mouth that you were looking for some possible areas of improvement in the hospital. I’ve got some data from SAMC’s in-home hospice program that might be useful.

I realize that you may not be familiar with the hospice program, so I also set up some meetings with a few people with a stake in the program. I’m hoping they can give you some context for the data you’re looking at.

Let me know if you need anything!

Sienna Pope,

Director of Medical Support Services

Hospice and Home Health Adverse Event Data 2014-2015

Per Vila Health policy, these figures include near misses as well as events that resulted in some level of harm or potential harm to the patient, for both home health care and hospice care patients. This is a summary of the data; a downloadable spreadsheet that provides all the data you will need for your presentation is also available below.

Unit – Year LOS Less than 7 Days IPU Admission Pain Level 7-10 More than 24 Hours Inadequate Symptom Relief More than 24 Hours
Hospice 2014 50 47 13 13
Hospice 2015 46 27 17 22

Quality improvement initiatives are a critical tool in the ongoing effort to improve patient care at health care organizations. But without data, many QI initiatives would fail — or the problem behind them might never be detected. That’s why data, and the dashboards that present data in a comprehensible fashion, are essential for QI efforts to succeed.

In this activity, you will assume the role of a quality assurance analyst at St. Anthony Medical Center. You will be offered both a dataset that you can use to outline a quality improvement initiative, and input from stakeholders who can help you contextualize the data.

 Educational Goals

After completing the activity, you will be prepared to:

  • Analyze data to identify a health care issue or area of concern.
  • Outline a QI initiative proposal based on a selected health issue and supporting data analysis.
  • Integrate interprofessional perspectives to lead quality improvements in patient safety, cost effectiveness, and work–life quality.

A number of quantitative methods are utilized to make decisions and recommendations in health care. Quantitative methods are used to analyze and predict the demand for patient services, to determine capital expenditures for facility and technology enhancements, and to guide the manager in implementing quality controls. Whether or not you are familiar with quantitative methodologies, as a manager, you are responsible for the outcomes of implementing the decision based on the method used.

Your agency or institution has noted a negative trend in profitability for a diagnostic imaging cost center over the past 4 quarters.

As a manager, you need to make some recommendations to take to your board of directors to reverse the negative trend.

Your first priority is to find a quantitative method to help you in making decisions.

Complete the following:

Choose a quantitative method (e.g., the decision tree model).

Describe the model that you are using.

Outline at least 4 proposed solutions to your board of directors, and analyze the strengths and weaknesses of each with regard to return on investment, break-even analysis, improvement in patient demand, improved patient safety and quality, and so forth.

Summarize how the decision-making method helped you make objective recommendations to your board of directors.

Health Promotion Plan Presentation. Nursing Assignments

Instructions: Health Promotion Plan Presentation

Top of Form

Bottom of Form

  • PRINT
  • Build a slide presentation (PowerPoint preferred) of the hypothetical health promotion plan you developed in the first assessment. Then, implement your health promotion plan by conducting a hypothetical face-to-face educational session addressing the health concern and health goals of your selected group. How would you set goals for the session, evaluate session outcomes, and suggest possible revisions to improve future sessions?

As you begin to prepare this assessment, you are encouraged to complete the Vila Health: Conducting an Effective Educational Session activity. The information gained from completing this activity will help you succeed with the assessment as you consider key issues in conducting an effective educational session for a selected audience. Completing activities is also a way to demonstrate engagement.

Note: All assignments in the course are based upon hypothetical individuals or groups.

Professional Context

Health education is any combination of learning experiences designed to help community individuals, families, and aggregates improve their health by increasing knowledge or influencing attitudes (WHO, 2018). Education is key to health promotion, disease prevention, and disaster preparedness. The health indicator framework identified in Healthy People 2020 helps motivate action in such areas as health service access, clinical preventive services, environmental quality, injury or violence, maternal, infant and child health, mental health, nutrition, substance abuse, and tobacco use.

Nurses provide accurate evidence-based information and education in various formal and informal settings. They draw upon evidence-based practice to provide health promotion and disease prevention activities to create social and physical environments conducive to improving and maintaining community health. When provided with the tools to be successful, people demonstrate lifestyle changes (self-care) that promote health and help reduce readmissions. They are better able to tolerate stressors, including environmental changes, and enjoy a better quality of life. In times of crisis, a resilient community is a safer community (ODPHP, n.d.; Flanders, 2018).

This assessment provides an opportunity for you to apply teaching and learning concepts to the presentation of a health promotion plan.

Demonstration of Proficiency

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:

  • Competency 3: Evaluate health policies, based on their ability to achieve desired outcomes.
    • Evaluate educational session outcomes in terms of progress made toward Healthy People 2020 goals and leading health indicators.
  • Competency 4: Integrate principles of social justice in community health interventions.
    • Evaluate educational session outcomes and the attainment of agreed-upon health goals in collaboration with participants.
  • Competency 5: Apply professional, scholarly communication strategies to lead health promotion and improve population health.
    • Present a health promotion plan to an individual or group within a community.

Preparation

This assessment builds upon Assessment 1 where you developed a health promotion plan for a hypothetical individual or group based on a topic list. You will resume the role of a community nurse tasked with addressing the specific health concern in your community. This time, you will present, via educational outreach, the hypothetical health promotion plan you developed in Assessment 1 to your chosen individual or group. In this hypothetical scenario, the presentation would be live and face-to-face. You must determine an effective teaching strategy, communicate the plan with professionalism and cultural sensitivity, obtain input on the value of the plan to the individual or group, and revise the plan, as applicable, to improve future educational sessions. To engage your audience, you decide to develop a PowerPoint presentation with voice-over and speaker notes to communicate your plan.

Remember that your first assessment (Assessment 1) MUST be satisfactorily completed to initiate this assessment (Assessment 4).

Please review the assessment scoring guide for more information.

To prepare for the assessment, you are encouraged to complete the Vila Health: Conducting an Effective Educational Session simulation. You may also wish to review the health promotion plan presentation assessment and scoring guide to ensure that you understand all requirements.

Adaptive Reuse

M3 Practitioner Application-Adaptive Reuse
Your M3 Practitioner Application will require devoted inquiry, assimilation, reflection, and
refinement in order to achieve the highest possible score. Specifically, you are to take the
scenario listed below, address it in full, and submit your work in accordance with the instructions
contained herein. Importantly, this is an independent assignment to be completed by you and
you alone.

Your M3 reading assignments include an article on adaptive reuse (see “Adaptive Reuse in the
Healthcare Industry: Repurposing Abandoned Buildings to Serve Medical Missions”), a practice
which has vast implications for healthcare strategy. It also represents a very good opportunity for
you to leave your “desks” and get out into the field to explore possibilities. Here’s the scenario:
A dentist desires to establish a clinic in or around your given marketplace and he has reached
out to you for assistance. He will serve as the sole dentist within the given clinic and he will offer
the customary array of services available at most any general dentistry practice. The dentist
desires operational efficiency and also takes opportunities where possible to make
environmentally responsible decisions. Having learned of adaptive reuse, he enlists you to
search the local marketplace seeking abandoned and available commercial buildings which
might be able to be converted into a dental practice through adaptive reuse. Your mission is to
identify three potential adaptive reuse candidates in your area, identifying their physical address,
describing them in detail, and ranking their potential to be converted successfully. The
candidates must be actual commercial properties which currently are for sale, so best to consult
commercial real estate websites and conduct good old-fashioned, boots-on-the-ground
investigations to identify prospects. Feel free to supply links in your text to real estate listings, if
available, or other resources to identify your selected candidates. If no such properties exist in
the immediate market, expand its boundaries until you find accommodations meeting desired
specifications.
Note that in the real world, a project such as this would represent quite the undertaking, requiring
possession of an extensive array of information (e.g., spatial needs, financial resources
available, location preferences, etc.). Such rich detail (and the time needed to make use of it)

isn’t available due to obvious course parameters, but you certainly have sufficient time to perform
a cursory examination within your local marketplace for purposes of identifying a few potential
candidates. That’s exactly what you must do, and through this, you’ll have a better understanding
of adaptive reuse.
The Submission Requirement
Attachments of any kind are prohibited and will carry no points value. Your submission must be
supported by 4 (minimum) to 10 (maximum) references, with at least 3 being from scholarly
academic journals. References may be prepared using the style guide of your choice (APA,); just
be sure to consistently use the selected style.
In presenting your work, identify the title, your name and student ID number, and submission
date at the top of your submission and supply the following sections, exactly as they appear
below, placing each heading in bold text:
• Introduction
• Adaptive Reuse Candidates
o Candidate 1
o Candidate 2
o Candidate 3
• Conclusions (one paragraph; summarize your candidate rankings here)
