Isabella is a 35 year old female who is currently employed with the military.  She has a history of post-traumatic stress disorder (PTSD) and it has just been confirmed at this community civilian clinic that she is pregnant with her second child. Physician confirm an unplanned pregnancy (G3, P1, T1, A1, L1), and her husband is currently deploy overseas in the military.
(G3, P1, T1, A1, L1) = (G3 – 3 pregnancies, P1 – 1 Pre-term delivery (before 37 weeks), T1- 1 Term Delivery, A1 – 1 Abortion (Both Surgical and Miscarriage before 20 weeks), L1 – 1 Living Child)
Learning Outcomes:

Firstly, conduct focused assessments in a primary care setting to evaluate patient status.
Secondly, identify factors which may influence or trigger changes in mental health to prioritize plan of care.
Thirdly, communicates with patient and support system in a manner that illustrates caring and reflects cultural awareness to establish rapport to which addresses the patient’s psychosocial needs and safety.
Fourthly, collaborate inter-professionally in a primary care setting to optimize patient outcomes.
