Root Cause Analysis (RCA) and Failure Mode and Effects Analysis (FMEA)

RCA and FMEA

A. Explain the general purpose of conducting a root cause analysis (RCA).
1. Explain each of the six steps used to conduct an RCA, as defined by IHI.
2. Apply the RCA process to the scenario to describe the causative and contributing factors that
led to the sentinel event outcome.
B. Propose a process improvement plan that would decrease the likelihood of a reoccurrence of
the scenario outcome.
1. Discuss how each phase of Lewin’s change theory on the human side of change could be
applied to the proposed improvement plan.

C. Explain the general purpose of the failure mode and effects analysis (FMEA) process.
1. Describe the steps of the FMEA process as defined by IHI.
2. Complete the attached FMEA table by appropriately applying the scales of severity,
occurrence, and detection to the process improvement plan proposed in part B.
Note: You are not expected to carry out the full FMEA.
D. Explain how you would test the interventions from the process improvement plan from part B
to improve care.
E. Explain how a professional nurse can competently demonstrate leadership in each of the
following areas:
• promoting quality care
• improving patient outcomes
• influencing quality improvement activities
1. Discuss how the involvement of the professional nurse in the RCA and FMEA processes
demonstrates leadership qualities

Nurse Management Orientation Program Proposal

Management Orientation Program Proposal
You have the opportunity to create a proposal to present to the leadership team for the orientation of new nurse managers. This project will include the creation of an orientation using leadership and management topics that will be selected to support the nurse manager as he or she transitions into the new role. This project will compile specific topics, resources and facilitators to support a new nurse manager as they get acclimated to the organization or unit. APA citations expected.

Your proposal should include:

Introductory and concluding remarks
Identify 8 specific topics to support the new nurse manager
Learning objectives for the lesson(s)
Rationale for including each topic with a synthesis of professional literature
Recommendation of internal and or external materials for each topic
Recommendation of a facilitator for each topic
Rationale listing why the materials and facilitator(s) are the most appropriate choice for that topic
Two or more scholarly resources to support the rationales
Support your discussion and opinions with facts, relevant examples from personal nursing practice.

All submissions must have a minimum of two scholarly references to support your work.

Theoretical Bases of Nursing (Nursing Theory)

Nursing knowledge is classified in a variety of ways, one of which is Carper’s Patterns of
Knowing (Carper, 1978). Carper’s framework offers a lens through which the nurse can reflect
upon insights acquired through empirical, ethical, personal, and aesthetic knowledge (Carper,
1978). Through intentional reflection using Carper’s Patterns of Knowing, nurses can process
experiential learning and knowledge acquired through practice. The purpose of this assignment
is to reflect upon a specific practice situation and better understand the professional knowledge Criteria for Content
Think of a surprising or challenging practice situation in which you felt underprepared,
unprepared, or uncomfortable.
Select an important nursing issue/topic that was inherent to the identified situation.
Briefly explain the situation
Identify the nursing issue inherent in the identified situation
As a method of refection, use Carper’s Patterns of Knowing to analyze the situation. In your
discussion, address ONE of the following Patterns of Knowing:
What do you think was the underlying reason for the situation? (Esthetics)
What were your thoughts and feeling in the situation? (Personal)
What was one personal belief that impacted your actions? (Ethics)
What evidence in nursing literature supports the nursing importance of the identified issue?
(Empirical)
What new insights did you gain through this reflective practice opportunity? How will this apply to
your practice as a nurse practitioner? Be sure to use scholarly literature to support your position.

Stakeholder Presentation (Proposal)

For this assessment you will create an 8-12 slide PowerPoint presentation for one or more
stakeholder or leadership groups to generate interest and buy-in for the plan proposal you
developed for the third assessment.
As a current or future nurse leader, you may be called upon to present to stakeholders and
leadership about projects that you have been involved in or wish to implement. The ability to
communicate a plan—and potential implications of not pursuing such a plan—to stakeholders
effectively can be critically important in creating awareness and buy-in, as well as building your
personal and professional brand in your organization. It is equally important that you know how
to create compelling presentations for others’ delivery and ensure that they convey the same
content you would deliver if you were the presenter.
