You will be required to deliver the health and wellness plan to the family in Week 4. Faculty must approve the health and wellness plan prior to delivery.

The health and wellness plan must be a minimum of 10 pages (excluding the references). The plan should be written at a 5th-grade level, and bullet points are best (or short paragraphs). Pictures may be used, but pictures may not take up more than 2 of the 10 pages. The plan must address health and wellness for the entire family (child, adult, and older adult) and will be a total of 10 pages. The health and wellness plan is to be professional and appropriate for dissemination to an external body.
